Yes, OSHA states that they have to be trained and evaluated. See the OSHA Standard below...
"The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph"
There three requirements that trainers must have to be qualified to train your PIT operators. Those qualifications can be found in 1910.178(l)(2)(iii)
"All operator training and evaluation shall be conducted by persons who have the (1)knowledge, (2)training, and (3)experience to train powered industrial truck operators and evaluate their competence."
Each class is unique for each individual customers needs. For a single classification of equipment and between 8 participants, the classroom portion takes 3+ hours. The Hands on portion and evaluations depend on the unique needs of each individual operator. Those who have more experience usually finish this quicker, and those who do not have any experience take longer.
No, you are only required by OSHA to re-evaluate them once every 3 years. If things are missed during their evaluation, that would trigger the need for Refresher training. This is the OSHA minimum requirement, you can always hold yourself to a higher standard and retrain and re-evaluate sooner

"Humble yourselves before the Lord, and He will exalt you." James 4:10
We have experienced the humbling and the exalting of the Lord and can affirm the truth of James 4:10. As believers, those who have put our faith and trust in the Lord Jesus Christ, we believe we are called to imitate our Lord and Saviour, and we do this by intentionally trying to elevate, lift, exalt , encourage, and make better the world we live in and the people we come in contact with.
